Isolation

If you have COVID-19 symptoms or test positive for COVID-19 (by home test kit):

  • Step 1: You need to isolate yourself  from others (at your home) for 5 days and wear a well-fitted mask around others. NYS counties are no longer tracking home tests.  
  • Step 2: However, please stop the spread and let others know you cannot do your daily activities (even if you do not feel sick). NYS is using this self-report/attestation form of Isolation for employment and other needs to document your isolation. You can complete online, then print and send to your employer. If you don’t have email or a printer/scanner, take a photo from a cell phone, see if a friend with email and a printer can help you or contact your local library.  
  • Step 3: Please follow additional steps for isolation such as contacting those you have been around for the previous three days.  If you have other questions, please review the NYS site or contact Livingston County Public Health, 585-243-7270. 

If you were exposed to someone who is positive for COVID-19:

  • Please review guidelines provided by the CDC.  
  • If you need to report to your employer, NYS provides this  self-report/attestation form.  You can complete online, then print and send to your employer. If you don’t have email or a printer/scanner, take a photo from a cell phone, see if a friend with email and a printer can help you or contact your local library.
